Tax Information (Form 1095-C)
Important Notice: 1095-C Statement Available Upon Request
2025 IRS instructions for Form 1095-C now allow an alternative manner of furnishing statements. Employers are no longer required to automatically send Form 1095-C to individuals.
Form 1095-C is available for employees and former employees who were offered health coverage through Nassau BOCES for 2025.
To request a copy of your form please use one of the options listed below.
